Dave passed away peacefully at home on February 18, 2014. He is survived by his wife of 30 years Marcella; daughter Sherry Tom Sawyer; two brothers, John and Donald Olivia; stepchildren, Paul, Jim Anna, Mindy, and David; granddaughters, Candice, Jessica, Holly, and Danielle; and 11 great grandchildren in Kentucky.
